Meeting individual need and expectation

Every resident, from referral to discharge, is at the centre of his or her recovery process. The referral and assessment process offers each resident an opportunity to think about his or her history, goals and whether Littledale is right for them.

Assessment focuses on key areas including (but not exclusively): alcohol and/or drug addiction; mental health; physical health; criminal behaviour; family. The process is robust and some of the topics can be difficult to talk about, however, completing the assessment gives you and us a good overview of where you are now and where you want to be and whether or not both parties feel need can be met.

Once a decision has been made for you to be admitted to Littledale a treatment practitioner will be identified to work with you from admission through to discharge. Careful consideration will be given to who your treatment practitioner will be as we believe this needs to be the staff member that can best support your individual needs during your recovery journey at Littledale.

Once you have been admitted to Littledale your treatment practitioner will work alongside you to co-ordinate your care. Littledale uses a comprehensive care management system, which includes regular assessments and care plans to jointly agree treatment options, goals and review progress. This is a collaborative process and copies of care plans are discussed and shared with care managers and other identified individuals responsible for your placement at Littledale. On-going personal health needs will be met by registering with our local GP and dental services.

In addition to this when planning your personalised care package we incorporate Dr S Galvani’s modified Six Cornered Addiction Rescue System to support you in your on-going and continued recovery. This model identifies meeting personal need across six key areas and was specifically designed to ensure stability and security is built in to recovery plans.

Leaving Littledale

The Aftercare Service

Aftercare is provided as part of an integrated package of personalised support for everyone completing treatment. Access to the service is entirely voluntary. Those most likely to benefit from Aftercare are ex-residents who have completed treatment and actively engage with peers and staff after discharge.
